Repeated no-shows may result in dismissal from care.

5. Insurance
Zupan Chiropractic may be out-of-network with insurance providers. Patients are responsible for understanding their insurance benefits.
We do not guarantee coverage, reimbursement, or claim approval.
6. Memberships & Programs
Any wellness memberships, care plans, promotions, or specialty programs (including but not limited to nervous system reset programs or prenatal care plans) are subject to separate written agreements.
Membership terms may include:
Auto-draft billing
Defined minimum commitments
Usage limitations
Cancellation policies
